Appointments
Appointments are made by calling our main office at (512) 420-9900 ext 205 or 201. New patient appointments are scheduled for 40 minutes and return visits are 20 minutes.
New patients
We will spend a few minutes on the phone with you to gather necessary information for your visit such as your:
Address and phone numbers
Insurance information - Please have your card
available when you call.
Primary care physician's name or who referred you to our office
Sleep disorder symptoms
Please bring the following to your first appointment:
Completed questionnaire (downloadable below)
Insurance card (see our list below)
Driver's license / photo ID

Please Note:
BCBS HMO or HealthSelect, Evercare and
Physician Health Choice require an authorization
from your primary care physician. Please make this
request prior to your appointment with our office.
If you don't have authorization you will have to pay
the bill.
We charge $25 for cancellations with less than 24 hours notice or missed appointments.
Co-pays and past due balances are expected at the time of your appointment.
